Why Toemi

Two women embracing to represent relationship-centered healthcare and emotional support.

A diagnosis, hospital stay, or change in health can affect far more than a medical record. It can change routines, responsibilities, relationships, and what everyday life looks like at home.

Toemi was created around a simple belief: people deserve to feel supported not only while receiving care, but in everything that happens around it.

Because healing happens at home.

Healthcare doesn't end when you go home.

The Gap We See

Healthcare has made remarkable advances in medicine, technology, and treatment. Yet some of the most difficult parts of a healthcare journey happen outside the clinical setting.

A discharge plan still has to become a plan at home. Follow-up appointments have to be scheduled and kept. Medications have to be understood. Transportation has to be arranged. Families have to know what happens next. And caregivers are often left coordinating it all.

When those connections break down, people can feel overwhelmed and unsupported.

Toemi is being built to strengthen what happens between healthcare and home.

Our Story

Toemi began with lived experience.

After years spent working within healthcare—and experiencing healthcare from the other side as a family member and caregiver—our founder saw the same challenge from two very different perspectives.

Exceptional medical care can happen inside a hospital or clinic. But when someone goes home, families are often left managing appointments, medications, transportation, changing needs, unanswered questions, and the emotional weight of caring for someone they love.

The healthcare system wasn't necessarily missing more information. What was often missing was connection, continuity, and someone helping make sure the next step actually happened.

That realization became Toemi.

Today, we're building a company designed to strengthen the space between healthcare and home—supporting individuals and caregivers while developing new ways for organizations and communities to extend care beyond the clinical setting.

Because people don't stop needing support when the appointment ends.

Health is shaped by more than medical care.

It is shaped by the caregiver who notices something has changed. The family member arranging transportation. The provider coordinating the next step. The community resource filling a need. And the home where everyday life continues.

At Toemi, we believe better care happens when those connections are stronger.

People. Care. Community. Home.

All connected around what matters most—the individual.

What We Believe

Looking Ahead

We're building beyond the traditional boundaries of healthcare.

Toemi's first focus is strengthening continuity between healthcare and home—helping individuals and caregivers stay connected to the people, resources, and next steps that support recovery and well-being.

But our vision goes further.

Through Toemi Homes, we're developing a new residential living concept grounded in the belief that needing assistance should never require someone to give up beauty, dignity, choice, good food, meaningful relationships, or the feeling of home.

Together, these efforts reflect one larger vision:

A future where care doesn't stop at the clinical door—and home remains at the center of how people live, recover, and age.
